Children's Health Home Care Manager
Working under the supervision of the
Children’s Health Home Care Manager Supervisor, the Children’s Health Home Care Manager provides high quality, family driven comprehensive care management services to a caseload of children and families with diverse strengths and needs. The Care Manager, with the support of the Care Manager Supervisor, will lead efforts in care coordination, health promotion, and other child and family support in home, school and community settings.
Sensitivity to the cultural diversity of others
Understands the medical, psycho-social, educational characteristics and needs of the population served by the Agency
Thrives working independently and on a team
Provides outreach and enrollment, develops Plans of Care, and conducts continuing care management services with children and families
Coordinates, identifies, and secures services relevant to the Plan of Care
Completes documentation with a high level of quality and detail to meet and exceed Health Home and DOH requirements
Participates actively in trainings, meetings, and functions at the agency and in the community
Provides quality direct Care Management services to children and families in home, school, and community settings
Uses a strength-based approach to arrange for, educate about, and manage the coordination of medical care and other services as needed
Provides on-call availability to Health Home Program as assigned
Interfaces and networks with partnering Health Homes, providers, and agencies
Must obtain certification to administer CANS-NY within 30 days of employment; certification must be continuously maintained while in the position
Other duties may be assigned.
Bachelor’s Degree in Human Services, Nursing or related field required; Master’s Degree in Human Services or Nursing preferred and a minimum of one (1) to three (3) years related care management experience with persons with disabilities and/or chronic illnesses.
